Incident alert

The loading shovel at an asphalt plant reversed into a customer vehicle which had been parked in an undesignated parking area. When the loader collided with the customer vehicle, it shunted the vehicle backwards and the vehicle trailer made contact with the driver's head. At the time of the impact, the customer was standing behind their trailer checking their burner was on, and was not wearing a hard hat.

• Review site Workplace Transport Plan & ensure it eliminates reversing as much as possible.
• Always pay attention to and use your mirrors, reversing cameras and alarms
• Complete an all-round vision assessment of your vehicle and ensure action is taken to remedy any blind spots
• Always check for any unexpected changes in your work area.
