WHAT HAPPENED
        
        A contractor operating an ALVAC Kerb lifter was almost injured when the
vacuum cylinder became unscrewed from its mounting and fell to the ground.
The “safety chain” could not hold the weight of the cylinder and the 10 x 8
Conservation Kerb!
After inspection it turned out that the “safety chain” was not designed to hold the cylinder and kerb!

LEARNING POINTS / ACTIONS TAKEN
  
All cylinders are to be inspected between each lift to ensure secure connection
Any chains and lifting arms MUST be able to hold the combined weight. The
Safe working load of these should not be exceeded.
