TITLE
'The Fatal 6' - Struck by Moving/Falling Object - Falling Tool
COUNTRY OF ORIGIN

WHAT HAPPENED

During a dismantling of some fixed scaffolding by the onsite scaffolding contractor, a tool from a Jack hammer fell from the open end of scaffolding tube several meters to the ground below. It became apparent a third party contractor had previously placed the tool inside the horizontal scaffolding tube for storage.

The investigation is ongoing however it became apparent this is not a standalone incident and although this was raised several years ago with controls for elimination, the incident has occurred at least once previously. A full review of controls is being completed.

LEARNING POINTS / ACTIONS TAKEN

Initial remedial actions
- Safety concern communicated to contractors.
- A TBT generated as a means of informing all future scaffolding activities.

Best practice is to cap all horizontal scaffolding tubes where known personnel may be working. The use of these end caps reduces the likelihood of items being placed into an open end tube.
