Mixer truck roll away

WHAT HAPPENED

 

A concrete mixer truck rolled away, striking fixed structures causing severe damage.

During a concrete delivery to a construction site a concrete truck mixer was parked on a 1:20 (5%) incline undertaking a ground pour of semi-dry concrete.

During the pour the driver moved the vehicle forward a few meters. The driver exited the cab and returned to the rear of the truck and continued with the pour. Approximately 30 seconds after the driver exited the cab the truck started to roll forward down the incline.

The vehicle rolled along an internal site road before striking a house under construction, which prevented the truck rolling any further.  No injuries were sustained.

The incident is currently under investigation, initial findings indicate, no mechanical failure on the braking system and likely cause is that the park brake was not applied.

LEARNING POINTS / ACTIONS TAKEN

Before exiting the cab:

  • Stop and physically check that the park brake is applied and park brake light is illuminated.
  • Check dashboard display and ensure no visual alarms are displayed.
  • Listen for any audible alarms. Are any sounding?
  • Is the parking area flat? If not, can another area be used, or additional anti roll devices be used?
  • Trailers - There is a ‘roll away’ risk for trailers that have been uncoupled from the towing vehicle. Ensure trailers are parked on a level surface, with trailer brake applied
