WHAT HAPPENED
A recent incident occurred at a rail aggregate depot which resulted in the derailment of a class 59 locomotives and a 102 tonne box wagon. This was due to material falling onto the railway lines from the adjacent unloading ramp – see photographs.

Will you please ensure all personnel are fully aware of the importance of good housekeeping, that unloading ramps are stable and secure and all walkways within the rail sidings are free from any spillage and obstructions.
