TITLE
Safety device failures
COUNTRY OF ORIGIN

WHAT HAPPENED

A company has recently experienced several failures of safety devices, which included the following

  • Stop pull cord control boxes not resetting
  • Stop pull cords breaking
  • Alarms and beacons not working
  • Isolator boxes breaking

Due to the regular checks carried out on the equipment, the faults were detected before the equipment was relied upon in an emergency. If the faults had not been rectified, it is possible that at some time in the future, an operator could have suffered a serious injury or fatality.

The range of faults found on the safety equipment highlights the importance of carrying out regular checks and ensuring that any defects are reported. Your system ensures that any faults on safety equipment are quickly rectified.

LEARNING POINTS / ACTIONS TAKEN

  1. Your regular checks may save someone from serious injury. Do not let them become a tick box exercise.

  2. Always lock out and try out before working on equipment. Never rely solely on safety devices
