WHAT HAPPENED
        
        During routine maintenance at Wrotham Sand Pit, a failure to an isolator was found.  The nylon connector on the metal rod had worn and failed to correctly connect with the isolator switch as it was turned to the safe position!
The failure was discovered when the staff conducted a “test start! As is recommended in the procedures and the HSE Approved Code of Practice.

LEARNING POINTS / ACTIONS TAKEN
  
The failure was immediately investigated and the fault found.  This is NOT the first time this type of isolator has failed!
This highlights the need to NEVER trust isolators and ALWAYS test the equipment BEFORE any person works upon it!
