WHAT HAPPENED
The Chassis failure was not identified on pre-start safety inspections or any service engineers report.
The machine is 2 years old approx and has done 2400hrs.
It is not normal to expect the steel to break like this and the site investigation is unclear what has caused the failure.
Komatsu engineers say they believe the driver must have run over a large stone to cause this type of damage.

Pre-start safety inspection must be carried out thoroughly.
Chassis inspections must be carried out based on the manufacturer's recommendations.
