LOCATION:
READYMIX OR MORTAR PLANT
ACTIVITY:

SUB ACTIVITY:
READYMIX
ALERT STATUS:
Normal
DATE ISSUED:
12/05/2009
INCIDENT No:

TITLE
Panel Fire
COUNTRY OF ORIGIN

WHAT HAPPENED

A burning smell was detected when the Batch Panel was switched on. The panel was isolated and when the front covers were removed, smoke could clearly be seen inside. The circuit was protected by an oversize fuse.

Immediate actions:
An electrician was called and found that two wires had fused together which generated the heat to cause the potential ignition. Also a large amount of obsolete wires were found in the panel that had not been removed once they had been disconnected. The electrician also found that the faulty circuit was protected by a 10 amp fuse, when ideally it should have been a 3 amp fuse.

LEARNING POINTS / ACTIONS TAKEN

•Panels should be inspected as part of Statutory Electrical Inspections and examinations.
•Redundant wiring should be either removed in its entirety or terminated in fixed wiring terminals and labelled as spare.
•Fused ways should be labelled with the protection device current rating.
•Any electrical defect are acted upon immediately .
