TITLE
Mixer reverses into Bus
COUNTRY OF ORIGIN

WHAT HAPPENED

In a recent incident a truckmixer collided with a bus parked in a bus terminus.

To gain access to the delivery site the truckmixer needed to pass through the terminus, then turn around and reverse onto the customer’s site. As the driver began to reverse the truckmixer, he did not notice the bus parked at the stop and struck the rear offside corner of the vehicle with the discharge chute of his vehicle.

Although the bus was empty at the time and no one was injured, this was a public place and shortly afterwards there were pedestrians alighting from buses at other stops.

The consequences of this incident could have been far worse than the asset damage that resulted.

The driver reported the incident immediately to his Manager and supplied the attached photograph

LEARNING POINTS / ACTIONS TAKEN

• Reversing should be avoided or kept to a minimum where possible

• Ensure the area you are reversing into is clear prior to manoeuvre

•Do you have all round vision (mirrors set correctly, camera working, monitor clear)?

• Follow your training/reversing rules

•If unsure do not attempt manoeuvre

•Ensure the use of a competent trained banksman where possible

•Consider the safety of pedestrians

• Advise your line manager / company contact of any delivery sites where you feel there are increased risks, particularly where these may affect the general public.

• Make full use of the Near Miss Hazard Alert reporting system...only by knowing about potential risks can action be taken to avoid injury.
