TITLE
Wheel Rim Failure
COUNTRY OF ORIGIN

WHAT HAPPENED

An LGV trailer wheel rim failed whilst the vehicle was manoeuvring to be parked up.

A packed trailer had been loaded in the packing bay, and was proceeding to be parked up on site, when the O/S rear axle wheel failed and became detached from the vehicle.

The wheel was manufactured in 1998, so was approximately 15 years old. The wheel also had significant corrosion on, and around the weld that joins the wheel flange to the wheel rim.

LEARNING POINTS / ACTIONS TAKEN

An immediate fleet survey took place, and any wheels showing signs of corrosion on or around the weld, were replaced.

As wheels are date stamped the second phase is to replace any wheel that is over 10 years old regardless of condition.

Please be vigilant when inspecting wheels, and report/replace any that show signs of corrosion on or around the weld.
