A driver was clearing a blockage from his pigs ear, and whilst doing so climbed onto the second rail of the slump stand railing to gain a better angle. By taking this action the driver had exposed himself to the risk of a fall and potentially serious injury.

Key Findings

• There was spillage built up by the slump stand that was preventing mixer trucks from getting close enough to the slump stand to easily clear blockages.
• The driver had not been fully inducted to site and not completed the comprehension test to ensure he understood it.
• Driver did not identify the hazards associated with completing the task that way and had not seen the RAMS for the activity.
• The RAMS for the activity did not adequately cover the activity.
• There is no standard procedure for clearing a blockage in the pigs ear within the company.

• The spillage was cleared at the base of the slump stand and a high-powered hose installed to keep the area clean.
• The risk assessments were reviewed and delivered out to drivers.

• Ensure individuals when inducted at sites are taken through the comprehension test.
• Ensure any individuals undertaking tasks are aware of and understand the relevant risk assessment.
• Review risk assessments around clearing blockages in pigs ears and ensure that they adequately cover the activity.
