A driver was clearing a blockage from his pigs ear, and whilst doing so climbed onto the second rail of the slump stand railing to gain a better angle. By taking this action the driver had exposed himself to the risk of a fall and potentially serious injury.

- Driver did not identify the hazards associated with completing the task that way and had not seen the RAMS for the activity.
- The RAMS for the activity did not adequately cover the activity.
- There is no standard procedure for clearing a blockage in the pigs ear within the company.
