TITLE
Welding incident
COUNTRY OF ORIGIN

WHAT HAPPENED

An incident occurred at a Hanson sand and gravel site in Cassington, Oxfordshire which resulted in fire damage to static equipment.

During a maintenance activity, a down pipe was being welded to cover a hole which was previously leaking water. Just as the welding activity was nearing an end smoke was beginning to bellow from the top of the screen. The welder immediately turned on the main water pump which extinguished the flames that were starting to develop.

Damage to screens (are being replaced)

No injuries sustained to any individual

LEARNING POINTS / ACTIONS TAKEN

Actions

1. During a maintenance shut down, all pipes should be investigated to identify if they have a rubber lining. All pipes that have been identified as having a rubber lining should be clearly marked to ensure appropriate personnel are aware prior to future maintenance;

2. A safe working procedure should be developed with the assistance of relevant personnel. The procedure must provide the end user with easy to follow steps and ensure that a repeat of this incident is kept to a minimum.

3. Review and amend the risk assessment that was previously carried out on the activity.

Summary

Although this incident didn’t result in any injuries to people, it is important we emphasise the risk fire poses to the business.
