A contractor climbed into crusher to have a look at the skato plates as these were on the job list for the day. They were advised at the start of the day to return to the office after completing his first job for the isolation to be removed, the equipment for the next tasks would then be isolated, ready for working on. The contractor failed to follow the instruction and consequently, the crusher was not isolated when he entered.
KEY FINDINGS - Root Causes
Failure to identify hazards - Contractor did not check to see if crusher was isolated before climbing in to assess works required.
LOTOTO procedure was not followed
Complacency – The contractor showed lack of hazard awareness
Violation of given instructions - The contractor did not return to the office following the first task
Inadequate communication - Insufficient communication from site regarding specific task
