'The Fatal 6' - Isolation Failure -Successful Use of Try Out
COUNTRY OF ORIGIN

WHAT HAPPENED

Try-starting a mobile crusher showed isolation to be ineffective...

• During a non-routine maintenance task, two contractors had isolated the Mobile Crusher they were to work on.

• The operator of the crusher also placed their padlock on the hasp so that they could ensure proper isolation of the machine.

• The operator applied the appropriate LOTOTO principle (Lock Out, Tag Out, TRY Out) and try started the crusher using the control panel, which resulted in the crusher starting.

• The sites robust application of the ‘TRY OUT’ element of LOTOTO potentially saved a contractor from a life changing injury or worse case fatal incident.


The picture shows the correct type of isolator was fitted to the mobile crusher and the start controls used to Try Start.

LEARNING POINTS / ACTIONS TAKEN

ALWAYS Try-start, even though the isolator has been locked out in the OFF position.

Procedure states “After the item of plant has been isolated, but before it is worked upon, an attempt is made to start the item of plant to check that the isolation is effective.”
