Reporting fatalities, injuries or high potential incidents (HiPo's) will help to ensure that others can learn from your experience and take the appropriate action to prevent a similar incident on one of their sites. We are all 'Safer by Sharing'

The table below summarises the HiPos received by MPA since the 1st January 2024. By using the drop down search functions below you can search using
different criteria and specify the date range. Alternatively, you can scroll down the table and review them in date order.

Where additional information has been provided this will have been added to Safequarry as an incident alert you can click on the link to access this. If you have logged on, you can select HiPo’s to save to your personal Safequarry file.
