The driver had completed off-loading the lorry and had positioned the roller crane to its transport position at the front of the lorry bed. As the driver walked along the centreline of the open lorry bed, his foot went through the floor and he lost his balance and fell off the lorry.
KEY FINDINGS
- The weather was windy and the floor area where he fell was built up ground consisting of stone
- The driver had over 40 years’ experience in this type of work
- The trailer had recently had its 6 weekly inspection and the floor was reported to be in a satisfactory condition
- The floor showed no damage or holes following it being loaded at the dispatching yard
