TITLE
TRANSPORT - Fatal 6 - VIDEO - Readymix truck loses control on a corner and rollsover
COUNTRY OF ORIGIN

WHAT HAPPENED

Please watch and share this dramatic video of a loaded readymix truck that loses control whilst cornering, crosses the carriage way and rollsover.

It was very fortunate that nobody was seriously injured in this high potential incident, the 6 year old vehicle was a write-off. 

The video explains the background to the incident and makes recommendations on how this type of incident could be avoided.

KEY FINDINGS

  • The driver was familiar with the road
  • He admitted driving at excessive speed
  • He stated that he may have been distracted by some personal issues at the time of the incident
  • The rear view camera was very dirty obscuring any view.

The common driver errors that are the cause or contributing factors in rollovers 

  • Driving too fast for the road conditions
  • Failure to consider the loads centre of gravity and movement
  • Unsafe cornering or turning speeds
  • Attempting an abrupt and unplanned manoeuvre
  • Improper pressurised rear axle settings
  • Failure to follow the appointed route to the pour site
  • Driving too close to the haul edge
  • Not carrying out a MYSAFE assessment, taking the time to scan the jobsite prior to positioning for discharge.

Key questions for a driver to ask themselves, can they answer YES to the following?

  • Are you trained and competent for the vehicle you are driving?
  • Are you fit and alert?
  • Have you carried out your daily checks, are all safety devices in good working order?
  • Do you drive in accordance with the vehicle's limitations, the load being transported and the road/weather conditions?
  • Do you know the safest routes?
  • Do you avoid distractions?
  • Do you wear your seatbelt?

If not, THINK, should you be behind the wheel?
