TITLE
Fatal 1 - Defective isolator on a Warrior 1400 Powerscreen and the importance of following LOTOTO process
COUNTRY OF ORIGIN

WHAT HAPPENED

 

An isolation was required to allow a conveyor belt to be replaced on a Warrior 1400 Powerscreen. When following the LOTOTO process the isolation owner applied a hasp, padlock, and tag to the main isolator. However, during the try-out step it was discovered that turning the isolator only a few degrees allowed the engine to be started. The make of the isolator was Cole Hersee.

On identifying the issue the work was stopped, the equipment made safe, and the isolator will be replaced.

The images below show the isolator, the isolator with the hasp and padlock fitted, and operator demonstrating that the conveyor could be started when isolator turned a few degrees with hasp and padlock fitted.

  • Following the LOTOTO process in full is essential.
  • The Try-Out step in this case identified an issue with the isolator.
  • Use this briefing note to reaffirm the importance of LOTOTO process and in particular, the Try-Out Step.
  • Identify any Cole Hersee isolators on your site and test them to ensure they are in working order and that isolation can be carried out effectively. If not, replace them with one that meets our company’s specification.
