WHAT HAPPENED
A mixer truck driver reversed his vehicle back to slump platform to inspect the load.
The driver got out of vehicle and accessed slump platform with the vehicle drum turning.
On inspecting the load driver decided to clean away materials from the drum/ chute area of the truck. As the drum rotated around at speed the drivers hand became trapped between the ‘pig’s ear’ chute and the blades of the rotating discharge point of the drum.

LEARNING POINTS / ACTIONS TAKEN
• Ensure that risk assessments are carried out on any activity relating to slump platforms.
• A physical stop-block should be installed beneath all platforms to ensure that vehicles cannot park within a set distance of the slump platform which prevents personnel accessing this area.
• Mesh or steel plate should be installed on the inside of handrails to eliminate the
risk of personnel climbing on handrails.
