WHAT HAPPENED
This alert is issued following a previous alert regarding the failure of a drive motor and gearbox mounted at height.

• Ensure Planned Preventive Maintenance (PPM) is scheduled in line with manufacturer’s recommendations as a minimum
• In harsher environments and high usage, more frequent inspection may be required.
• Ensure all materials used in the PPM or breakdown repairs meet with the manufacturer’s specification, and any other technical standards that may apply.
• Where individual component parts require replacing consider the benefit of replacing the whole assembly to avoid the potential for backlash.
• Carry out assessments identifying areas where equipment is mounted / travels over people, consider fitting a secondary securing device.
• Secondary securing devices prevent a component falling from height if the primary securing method fails. This could be a safety wire or some other engineered method designed for this function. Secondary securing devices should be sufficient to withstand the shock loading that may occur if the primary securing method fails.
• Encourage employees to maintain vigilance and report concerns with machinery (for example: report strange noises / changes in operating performance as potential hazards).
