WHAT HAPPENED
After the main engine of a UK marine aggregate dredger was shut down while the vessel was alongside, the Duty Engineer was engaging the turning gear in order to turn the engine over as per normal operating procedure.
The pin for the turning gear had seized due to a sea water leak from the air cooler drain dripping on the pin causing it to become corroded.
When the Duty Engineer tried to release the pin (unaware that it was seized), he strained his left shoulder. The injury required medical assessment, which diagnosed rest and the use of painkillers.
Root Cause:
Inadequate inspection and maintenance regime in place to ensure the satisfactory working of the specified shipboard equipment.

Planned Maintenance Job Card added for the Turning Gear Pin, requiring inspection on a frequent basis and periodic lubrication.
Risk assessment and Safe Working Practice for the operation reviewed to ensure that the hazard of “engaging and disengaging the turning gear pin” is considered.
