WHAT HAPPENED
A haulier HIAB was waiting to be loaded with waste from the premix plant at Dagenham and had his arm and bucket raised to allow the FEL to place material into the body of the vehicle. He was then instructed to load himself, so attempted to manoeuvre around to the other side of the bay but FORGOT about the arm and bucket being raised and struck the support beam of the sand shed!
His alarm was sounding to warn of the raised arm and bucket – BUT HIS RADIO DROWNED OUT THE SOUND OF THE ALARM!

LEARNING POINTS / ACTIONS TAKEN
The photo shows the damage done to the shed. A structural engineer inspected the damage and passed it as safe. The haulier has had to pay for the inspection and repair of the shed and has been banned from all London Premix sites until he has installed a secondary warning device (flashing lamp) to his dashboard.
