WHAT HAPPENED
Whilst walking across the engine room an engineer slipped and fell down a short step causing a painful
back injury and possible fractured rib.

Corrective Actions \ Recommendations
The investigation and POI revealed a number of issues:
1.The engineer was in a hurry
2.The handrail could have been positioned better
3.The handrail should have been used as he went down the steps
This incident shows the importance of always having 3 points of contact when climbing down ladders or
travelling down steps. As you can see from the photo, the severity of injuries doesn’t always depend on
the distance fallen. The engineer needed a considerable period of time to recuperate with numerous
medical visits.
Onboard management are asked to check all handrails for suitability and visibility and where appropriate
carry out a risk assessment to develop corrective action for all potential slips trips and falls i.e. painting of
handrail in bright colour e.g. yellow, additional handrail to be added, standard of footwear, tidying up of
trailing cables etc. Crew are reminded to exercise their duty of care when travelling around the ship.
