WHAT HAPPENED
An employee was knocked down by a reversing lorry whilst supervising a subcontract activity. The employee was using a mobile telephone at the time and was distracted from what was happening around him. The reversing lorry was not under the control of a banksman who was some distance away and the vehicle had a defective reversing camera and reversing alarm. The employee was extremely fortunate in that he was able to roll out of the path of the lorry wheels and he only suffered minor scratches and bruises. This incident could easily have resulted in a fatality.
There were 4 principal factors that contributed to this accident: -
• The banksman was not present;
• The lorry reversing camera was
defective;
• The lorry reversing alarm was defective;
• The employee was distracted by a
mobile telephone.

• Ensure that reversing lorries are
supervised by a banksman at all times
when reversing in a working area;
• Ensure that lorry reversing cameras
and reversing alarms are working and
used;
• Ensure that vehicle maintenance,
inspection and defect reporting
systems are effective;
• Ensure that persons are not
distracted by mobile telephones and
only use them from a place of safety.
