WHAT HAPPENED
During clearance of returned asphalt from a temporary stockpile at Dagenham Depot, the FEL driver decided to dig under the material to get extra amounts into his bucket and in doing so, reduced the level of the surface. He managed to unearth and sever an 11000 volt HV cable. Luckily he was unhurt!
The cable was known to be within the vicinity, but was thought to be around 4 foot below the surface as are the other HV cables on site. The operator did NOT know of the cable, nor was it on the sites buried services diagram

The batch plant was out of action for several days
LEARNING POINTS / ACTIONS TAKEN
This highlights the need to always know the depth and location of ALL underground services and to adequately risk assess BEFORE stocking material close to transformers or buried services and to ensure operators are briefed accordingly.
