WHAT HAPPENED
On a routine inspection of this site it became apparent that contractors and Hanson personnel failed to identify a confined space, or eliminate reasonably foreseeable hazards to remove the confined space risk.
Before working in any excavation ensure the following are controlled:
Carbon Monoxide ingress, as it is heavier than air. Ingress of material from drains.
Ensure emergency evacuation procedures are in place.
Ensure RA & SWP are adequate.
Ensure competency.

LEARNING POINTS / ACTIONS TAKEN
Remove the JCB to eliminate the possibility of fume flowing into the excavation.
Ensure RA & SWP are suitable, sufficient and relate to the task.
Arrange for HC management to attend Managing Confined Space training
