WHAT HAPPENED
An employee from the recycling plant had parked his car behind the FEL to clock out at the end of his shift.
Points noted:
The driver of the FEL claims he did not see the vehicle and reversed to turn his machine around.
The sun had been shining on the camera causing glare.
It has been common practice for vehicles to stop any where in the vicinity to clock out.
It was common practice for the FEL to be parked next to the weighbridge.
The parking area was full .
The FEL driver once reversed was going to drive against the flow of traffic.

LEARNING POINTS / ACTIONS TAKEN
Purchase a clocking in machine for recycling.
Reversing cameras are an aid and not to be relied upon as sole use when reversing.
Ensure vehicles park in the designated areas.
Remove unnecessary plant from the parking area.
Review the traffic management RA and SWP for site.
