Employee breaks wrist in fall from platform/ladder.

There is no edge protection on the platform. The ladder does not extend 1 metre above the platform and presents a trip hazard when on the platform.
LEARNING POINTS / ACTIONS TAKEN
-Ensure all exposed platforms have guard rails
-Ladders should extend 1 metre above landing points
-Place ladders at 1 in 4 angle
-Do not overload ladders
-Maintain 3 points of contact
-Ensure boots & ladder rungs are clean
-Use tool belts to avoid carrying
-Inspect ladder feet, stiles & rungs for damage before use
-Place ladders on firm ground & ensure they are level
-Tie off ladders where practicable.
