WHAT HAPPENED
What happened?
•A pair of crane crabs were suspended in preparation for a job in BB 22.
•The nuts were not properly tightened on one of the crabs, allowing the wheels to buckle.
•On the other crab the bolts were not well packed, so that the nut could not be properly tightened, and the wheels could buckle.
•At the start of the job the crabs were not checked for correct mounting.
•During the job a load of 2200kg was suspended from the crabs. The load shifted the wheels, so that the load could have fallen.

These nuts were not properly tightened, so that the wheels were leaning under load.

There is only limited thread per bolt, so that it is important to pack the bolts to the maximum. One washer was missing so that the nut reached the shoulder and could never be fully tightened.

Recommendations:
•When starting a job, always check the material to be used for testing, positioning, execution.
•Do not assume that a situation is safe, but determine for yourself whether it is safe. It is up to you to decide whether you find something is safe or hazardous.
•Discuss the working methods with your colleagues in advance.
