WHAT HAPPENED
A contractor employed on the installation of new re-feed bins was using the sites Manitou Telehandler to ease a new bin into position for bolting down. The lifting hook suddenly failed causing the bin to drop suddenly.
• Luckily the bin was within the confines of a close fitting structure and only dropped a few inches into its intended resting place.
• The Crane Jib attachment had been tested by Allianz and a current test certificate was held on site. The total load on the hook was around 50% of the rated capacity of the Jib.
• Due to the design of the Jib it would not be readily obvious through visual inspection that there is a problem with the hook and eye.

• The attachment must be disassembled and thoroughly inspected by a competent person on a regular basis and if there is any doubt do not use.
