WHAT HAPPENED
As a result of an incident investigation following the failure of a load bearing part, it was apparent that safe working load (SWL) identification could have misled the site operatives to work above the SWL for the equipment.
Although the lifting equipment was rated at 2 tonne, the beam it was working off was only 1 tonne.

LEARNING POINTS / ACTIONS TAKEN
A lifting hoist SWL must not exceed the SWL of the beam to which it is attached. Any hoist with an SWL above the SWL of the beam to which it is attached, must be down-rated and clearly marked.
