WHAT HAPPENED
During a planned training session operators were being shown how to dress an abrasive wheel using a “Clarke Star Dresser”. As the trainer attempted to dress an abrasive wheel the dresser device disintegrated ejecting its mechanical parts. Fortunately no one was hurt from this incident and everyone was wearing correct PPE.
• During investigation it was concluded that the disintegration of the dressing device may have been caused by a design defect with the interlocking pin or its components, which may have become loosened during operation. This defect could not be identified prior to use.

Therefore:
• If you do decide to dress an abrasive wheel disc, inspect any dressing equipment fully before use and wear the correct PPE identified from your risk assessment prior to working.
