WHAT HAPPENED
An Agency Able Seaman joined a UK marine aggregate dredger on the morning of a crew change. He was subsequently told to wait in the crews mess for the on-signing crew.
The IP met the on-signing captain on the bridge where he was briefed on the operation of the vessel and the minimum safety requirements expected of him. The IP was told not to start any work until a full induction had taken place.
The IP was briefed in the afternoon by a company crew member of the PPE requirements for working on-board the vessel and all routine hazards for walking around the vessel. The IP was then taken on his familiarisation tour of the vessel for induction purposes.
During this walk around the IP lost his footing on a rogue stone on the safety walkway put out his hand to steady himself and caught his finger on the middle safety railing causing it to bend back and dislocate.

Vessels to conduct a Safety Briefing discussing the above incident highlighting the importance of conducting a good briefing with new unfamiliar crew members before conducting a tour of the vessel, reminding of the importance of being vigilant when conducting routine activities.
