WHAT HAPPENED
A gas bottle cage was lowered into position, close to a work area, in preparation for some cutting and welding work.
Two wooden batons were positioned on the ground to spread the load of the cage and provide a level footing.
However, the ground conditions had not been fully taken into account and the batons settled into the mud causing the cage to tilt and fall.
The cage toppled forwards and caused multiple fractures to the Contractor Supervisors leg.

Risk assessments should cover preparation for a task as well as the task itself.
When lifting or moving equipment, prior consideration should be given as to whether the load will be stable when placed into position.
Checks should be carried out to ensure that the load is stable before removing the securing apparatus.
A suitable exclusion zone should be established during lifting and placing operations
Designs that are stable on level hard surfaces may not be so on any slight slope or uneven ground. The cage should have been made secure or alternative options considered.
