WHAT HAPPENED
A contractor was attempting to cut a rubber liner to fit a chute. As he proceeded to cut the material, the "Stanley knife" slipped over the edge of the steel rule and sliced his thumb.
The contractor failed to follow the safe system of work issued by his own company, was experienced, had been inducted and had even been witnessed working safely earlier in the day.
The injured person took a shortcut that ended up requiring hospital treatment.
