TITLE
Potential Mixer Drum Entanglement
COUNTRY OF ORIGIN
WHAT HAPPENED
A serious incident occurred recently during the washing down of a mixer drum following discharged concrete. It is believed that the cause of this incident was the type of tap (see picture ).
The design of the tap, and dependant on where an individual washes the blades, could result in the tap becoming wedged in the mixer blade, pulling the hose and potentially the individuals hands/arm into the mixer.

LEARNING POINTS / ACTIONS TAKEN
These types of taps should be replaced with twist on/off taps.
Mixers should only be washed in discharge mode.
All 'guarding' of mixers should be assessed to ensure that it is adequate if a hose should become trapped.
