WHAT HAPPENED
An Articulated dumper cab turned over whilst attempting to reverse down a slope with the cab at 90 degrees to the rear body. The driver suffered bruising to his right thigh and was prevented from further injury as he was wearing his seatbelt.
Whilst attempting to reverse down the slope the driver had his cab angled at 90 degrees to the trailer section which was positioned down the slope. Due to the width of the flat area at the top of the slope and the area being driven and turned on several times, depressions had appeared.
The front left hand side wheels were just off the flat area and on to the slope. The front left wheel went into a depression causing the cab to dip, due to this the weight of the rear body and its load caused the rear to drag the cab over and then slide approx 6 metres down the slope.

LEARNING POINTS / ACTIONS TAKEN
This accident could have been prevented by:
- the tip area being a more suitable grade for the operation and the operation better planned
- a more stringent inspection regime of the tip area by site managers, supervisors and drivers
