WHAT HAPPENED
An error of judgement led to the skip of an Articulated Dumper overturning while tipping materials on to a stockpile.
The operator mistakenly thought he was helping the shovel driver by reversing the dumper up the face of the stockpile to tip in order to save the shovel pushing up.

LEARNING POINTS / ACTIONS TAKEN
•Never reverse a dumper up the face of a stockpile
•Always reverse square on to stockpile faces
•Leave enough clearance so the skip does not make contact with the stockpile face when being raised
•Keep all the wheels on firm level ground when tipping
•Always turn and visually check that the skip is level before tipping, particularly with ADT’s, (the cab can be sat level with the rear bogie and skip sat at an unsafe angle).
•Never take short cuts or alter your method of working unless it has been approved by a competent person
