WHAT HAPPENED
Whilst, a mixer truck was delivering material to a contractor’s site, an employee of the contractor approached the rear of the truck in a 1 tonne front loading site dumper.
The operator of the dumper became confused with the dumpers controls and the dumper moved inadvertently forward bending the mixer trucks delivery chute winding handle. The operator then in error put his foot back on the throttle sending the dumper further forward bending the motorway maintenance sign and its frame.

LEARNING POINTS / ACTIONS TAKEN
No injuries were sustained from this incident because the truck driver was standing in the appropriate discharge position to one side.
The truck driver reported the near miss and clearly demonstrated that by following a suitable and sufficient Safe System of Work; his health and safety was protected, even in the event of an uncontrolled unsafe act by another.
