WHAT HAPPENED
An accident occurred when a driver was alighting from his Truck-mixer vehicle in day light, on a dry day and on flat level ground.
Whilst coming down from his cab, the driver caught his foot on the bottom step, twisted and fell to the ground.
The Panel of Inquiry could only conclude that the accident happened because the driver did not maintain 3 points of contact until both feet were firmly on the ground before turning to complete his
other tasks.
The bottom steps, have been damaged on some vehicles and removed. Furthermore, some steps have been taken off before they get damaged because of the cost of replacement.

- Drivers must maintain 3 points of contact
- Ensure vehicle steps provided by the manufacturer are on the vehicle
- Ensure all vehicle steps are clean, free from debris and collision defects
