WHAT HAPPENED
The tragic deaths of two maintenance fitters has focused attention on a range of key issues involving the interaction of heavy mobile plant and small soft-skinned vehicles. The pdf that is attached to this incident alert is a presentation that reconstructs the lead-up to the double fatality and gives an insight into the multiple learning points for quarry operators, concentrating on organisational behaviour; behavioural safety; heavy mobile plant design; and the huge consequential costs to an organisation.

Segregate heavy mobile plant & transport from people & light vehicles, wherever possible
Ensure conspicuity
Ensure all-round vision, but avoid driver sensory overload
Site rules to be well communicated and enforced
Enforce strict radio protocol
All vehicles to have clear, large, I.D. markings
Review driver training – never turn into ‘blind area’
Review driver distractions (mobile phones; iPods; newspapers; etc)
