WHAT HAPPENED
A haulier collected grit stone dust from a quarry and attempted to tip off at an Asphalt plant. The driver recognised that his trailer was unevenly loaded and requested assistance from the site to counter balance it.
On the drivers request, additional material was added to the tipper and when the driver attempted to tip a second time the load did not release and at full extension the tipper ram failed resulting in the tipper body rolling over.
The initial investigation has established that the grit stone dust had a high moisture content, and the load was not evenly distributed. A full investigation and Panel of Inquiry has been scheduled and its findings will be communicated.

Learning Points:
1. Only load materials that are suitable for delivery.
2. Assess the suitability of your loading equipment in relation to the size and height of tipper bodies to ensure they can load safely.
3. Ensure every load is inspected at the loading site, never release an unstable load onto the highway.
4. Never add additional material to balance loads if it impacts on the maximum load capacity of the tipping system.
This is not the first occasion that an articulated tipper has rolled over at a delivery point. It is essential that due care and attention is taken at both the loading point and tipping point. Never release an unsafe load on to the highway; always inspect the load before dispatch; always ensure the delivery tipping point is suitable and never add additional weight to an unstable load or incorrectly loaded wagon.
