WHAT HAPPENED
Whilst putting out the gangway, the Able Seaman pushed the support stanchion which gave way suddenly causing him to fall into the water alongside the ship.
Fortunately he was retrieved from the water quickly. Tide speed and water temperature could have easily made this incident far more serious.

LEARNING POINTS / ACTIONS TAKEN
Causes and Action:
•Failure to follow the Safe System Of Work – which clearly stated that lifejackets and harnesses should be worn when carrying out this task as well as placing a safety chain across the gap when during the activity.
•Failing to follow and review the Risk Assessment for the task.
•Maintenance procedures need to ensure that moving parts are in good working order.
