The following Critical Incident occurred at an asphalt site in August.

Initial investigations indicate that the third party hotbox wagon was loaded at the asphalt plant. The driver parked just beyond the designated sheeting area and failed to apply their parking brake. Whilst performing checks on the wagon prior to leaving the site the vehicle began to roll away.

The driver jumped down from the vehicle on the passenger side and ran in front of the moving vehicle to try and access the cab and stop the vehicle. The driver was unsuccessful in stopping the vehicle which came to rest after colliding with the boundary fence.

Images below show where the vehicle crashed into a barrier on-site and image caught by camera as the vehicle rolled away. 

• Always park in the designated parking areas, where provided
• Always ensure parking brakes are applied before exiting the vehicle
• Avoid parking on a gradient, however if not possible, ensure the wheels are also chocked and vehicle is stable
• DO NOT put yourself in the line of fire

An Investigation team has been formed and are currently investigating this incident to understand root causes and recommend corrective actions and learnings
