WHAT HAPPENED
The British Constructional Steel Association has been made aware of another incident involving a Mobile Elevating Work Platform Genie Z45/25J.
This is another incident of the tilt action of the basket being activated automatically with the operator unable to stop or override the action, resulting in the basket tilting to 45º with the operator in the basket at height.
A previous alert from the BCSA highlighted this concern but as yet they are not able to give any specific advice or guidance on what is needed to prevent any further occurrences as the manufacturer, supplier and the Health and Safety Executive have not provided any information of their investigations into what is known as a growing number of similar incidents.
The Genie Z45/25J was in use by netters on a project in London on Monday 10th November 2014. As the work was taking place at height the basket started tilting without levers being operated. The operative lowered the boom and supplier was contacted to send a fitter to examine and repair the fault.
The supplier’s fitter arrived Wednesday 12th November 2014 and repaired MEWP. Shortly after the supplier’s fitter left the site the machine was used for approximately 20 Minutes when the same fault occurred and machine was quarantined. The machine was removed from site on 13th November 2014.

Fortunately again with this incident no injuries have been reported as the operators were all experienced and following the good practices for MEWP operation (especially ensuring they had secured their harness to the anchor points).
