An isolation was required to allow a conveyor belt to be replaced on a Warrior 1400 Powerscreen. When following the LOTOTO process the isolation owner applied a hasp, padlock, and tag to the main isolator. However, during the try-out step it was discovered that turning the isolator only a few degrees allowed the engine to be started. The make of the isolator was Cole Hersee.
On identifying the issue the work was stopped, the equipment made safe, and the isolator will be replaced.
