WHAT HAPPENED
An accident occurred at West Drayton recently where an operator was climbing down from his Volvo Loading Shovel and the steps gave way.
After investigation two retaining bolts had previously sheered and the incident occurred when the third bolt snapped, causing the steps to tip back. Luckily the forth retaining bolt, which was also loose, remained in place, otherwise the complete steps unit would have detached from the body of the vehicle.
As it was, the operator lost his balance and fell 1.5m to the ground, twisting his ankle. This incident resulted in the operator being off work for over two weeks.

This accident highlights the following issues:
• All operators should regularly inspect the security and condition of all access/egress steps on all Mobile Plant
• Site Mobile Plant Inspection schemes should include a section to ensure compliance with these checks
