TITLE
Fatal 5 - TRANSPORT - RUNAWAY - A concrete mixer truck rolled away, striking and injuring the driver.
COUNTRY OF ORIGIN

WHAT HAPPENED

 

During a delivery to a client’s CDM site a concrete mixer driver exited the vehicle cab. Whilst the driver was out of the cab the mixer truck rolled forward striking the driver before finally coming to a rest against a tree.

The incident is currently under investigation and although the details as to the cause of the ‘roll away’ are yet to be established, all are reminded that prior to a driver exiting a vehicle.

  • the handbrake is applied.
  • vehicle is taken out of gear (on automatic vehicle ‘Park’ is selected).
  • engine switched off (unless operational requirements prevent this).

Further Consideration:

  1. Drivers to always check the hand brake warning light is illuminated, indicating the hand brake is engaged when parked.
  2. Any Parking area, is it flat? If not, can another area be used, or additional anti roll devices be used?
  3. Trailers - There is a ‘roll away’ risk for trailers that have been uncoupled from the towing vehicle. Ensure trailers are parked on a level surface, with trailer brake applied.
  4. Please share the 'Safe Stop' information with your colleagues
