WHAT HAPPENED
During a delivery of concrete to a customers construction site, a driver was asked to discharge his load into a skip. This was to allow an excavator to 'manage' the material and allow the delivery point and area of use to be separate.
During discharge the excavator bucket moved the skip causing the drivers thumb to be crushed between the chute handle and the skip edge. The drivers thumb at the knuckle was severed and despite the driver being transferred to a specialist hospital, it could not be reattached.

LEARNING POINTS / ACTIONS TAKEN
- Mobile plant should never be operated in the close proximity of personnel.
- Operatives are reminded of our Zero Harm pledges; specifically ‘Stop and ask if we feel at risk' - if in doubt suspend the task and contact the site manager.
- Always ensure a safe distance from the delivery chute and the material receptacle / intended destination.
